CJA5 – Birth Registration (Labour & Delivery Visit)
Role: Clinic Staff, Clinic Manager / Nurse-in-Charge, Midwife
Use when: Recording a complete birth event in the Immunization Information System (IIS) — including mother–baby linkage, barcode generation, documentation of birth-dose vaccines (BCG 0, HepB 0) and Vitamin K, and closing the pregnancy record.
Always start the visit under the mother. Birth Registration is part of the Labour & Delivery Visit, not a standalone record.
🧭 Steps
1️⃣ Start the Labour & Delivery Visit
From Patients → Search, open the mother’s record.
Select Start Visit → Labour & Delivery Visit.
Confirm the correct facility and session.
2️⃣ Record Maternal Vital Signs
Enter HR, BP, Temp, RR → review for Normal / High / Low.
Click Complete to save.
If unable (emergency/quick delivery): select Remove to skip the Vital Signs task.
3️⃣ Add Birth Registration
From Visit Actions → Add → Birth Registration
Enter:
Date & Time of Birth
Delivery Method, Outcome, Sex
Gestational Age
Apgar Scores (1, 5, 10 min)
Birth Weight, Length, Head Circumference
Multiple Births if applicable
4️⃣ Record Birth Doses
Tick or enter:
BCG 0 (≤ 24 h, ID left arm)
HepB 0 (≤ 24 h – ≤ 7 d, IM right thigh)
Vitamin K (≤ 6 h, IM left thigh)
Record Lot / Batch # for each.
If any dose is not given, record the reason for follow-up.
5️⃣ Save → Generate Barcode
Click Save / Complete.
The system automatically:
✅ Creates the newborn record
✅ Generates the IIS barcode
✅ Links mother and child
6️⃣ Neonatal Visit Prompt
Start a Neonatal Visit if the baby requires its own visit outside of the birth vaccine record.
7️⃣ Complete Pregnancy Record
After delivery, complete a Pregnancy Completion form to close the pregnancy and update gravida/parity counts.
From Add → Pregnancy Completion / Termination
Enter:
Outcome of Pregnancy: Live Birth / Stillbirth / Miscarriage / Induced Abortion / Unknown
# of Pregnancies (Gravida): Total pregnancies to date
# of Births (Term): Full-term deliveries
# of Preterm Births: < 37 weeks
# of Abortions: Include miscarriages (spontaneous + induced)
Births – Living: Number of living children
Click Complete to save.
Key Reminders
One Birth Registration per infant
Each Save creates a unique barcode
For stillbirths: no barcode / newborn record — data remain under the mother’s visit
For false labour: Outcome = No Delivery / Not in True Labour → End Visit; add Birth Registration later if delivery occurs
