Patients

Purpose
The Patients module is the foundation of the Immunization Information System (IIS). It provides tools to find, review, and register individuals so that each person maintains a single, accurate IIS record across all clinics, outreach sites, and facilities.
This record is central to all other workflows — including vaccination sessions, care pathways, and reporting. When managed correctly, it ensures:
Safety – preventing duplicated or missing vaccination records.
Continuity – allowing patients to be recognized across facilities.
Data quality – ensuring every administered vaccine links back to the correct person.
Professional Standards
Accurate and Timely Documentation – Patient and immunization data are recorded at the point of care, reducing transcription errors.
Continuity of Care – Each patient’s longitudinal record follows them across sites, enabling informed decision-making at any facility.
Confidentiality and Privacy – All access is controlled through authenticated logins and role-based permissions.
Professional Accountability – Every entry is traceable to the author, reinforcing ethical and legal responsibility for documentation accuracy.
Equity of Access – The IIS helps ensure that marginalized or mobile populations are not lost to follow-up by maintaining a single unified record.
Summary
From the Patients menu, authorized users can:
Search for existing records to avoid duplicates
Register new patients when no record exists
Access the Patient Dashboard for a real-time overview of patient activity
Open the full View Patient record to review demographics, identifiers, and complete care history
Re-compute care pathways or update vaccination plans when data change
This module forms the entry point to all clinical workflows. Each patient’s IIS record links directly to:
Vaccination Sessions – for dose administration and stock usage.
Care Pathways – for ANC, Child Health, and Routine Immunization tracking.
Reporting Dashboards – for accurate population-level analysis and coverage reports.
By using this module as intended, facilities maintain one authoritative IIS record per person, supporting safe immunization practices and high-quality data for national planning.
