The Audit Logs provides Clinical Managers with a read-only summary of actions performed by users within their facility. This page supports data accountability, oversight, and traceability of clinic-level activities, such as patient registration, vaccination sessions, and stock adjustments.

The Audit Trail allows Clinical Managers to:

  • Verify proper handling of records and clinic data

  • Review staff activity logs for quality assurance

  • Support compliance with Ministry of Health and UNICEF audit requirements

Managers can observe and report activity but cannot make system-level changes.

Although the Audit Trail (Manager View) is read-only, it provides Clinical Managers with oversight of user activity that directly impacts data quality and vaccine accountability. Managers can confirm that data entries are being completed correctly and follow up on irregularities reported by staff or observed in reports.

Situation
How the Manager Uses the Audit Trail

A nurse reports that a child’s vaccination record disappeared or looks incorrect.

The manager searches recent audit entries for that patient to confirm whether a record was updated or deleted, and by which user. This helps determine if it was a system error or a data entry mistake.

A stock balance in the cold chain inventory does not match the expected count.

The manager reviews audit actions under “Stock Management” to see if extra doses were issued, adjusted, or marked as wastage.

A user reports that saving a vaccination session failed or froze.

The manager filters by “Outcome = Fail” to confirm the failed action and share the timestamp with the system administrator for technical follow-up.

During a routine data quality review or external audit.

The manager views recent “Create” and “Update” actions to demonstrate staff activity and confirm that records are being actively maintained.

Accountability and training.

The manager’s Audit Trail page is a simplified, read-only version of the administrative audit interface.

It displays a summary table of recent audit entries with the following columns:

The table displays system and user activity, including:

  • Action: What operation was performed (e.g., Execute, Login, Read)

  • Event: The system area related to the activity (e.g., ApplicationActivity, SecurityAlert, UserAuthentication)

  • Outcome: Whether the action succeeded or failed (Success, Minor/Moderate Fail, Serious Fail)

  • Timestamp: When it occurred

  • Actor: Which user or process triggered the event (e.g., localhost, anonymous, or a named user)

This view allows a Clinical Manager to confirm successful logins, identify failed system actions, and monitor user authentication events for their facility — ensuring both clinical and technical workflows are functioning correctly.

  2. Select a User from your facility to view their recent audit activity.

  3. Review the audit summary:

    • Confirm expected activities such as record creation, updates, or deletions.

    • Review timestamps and outcomes to verify actions.

  4. If something looks unusual:

    • Note the timestamp, user, and action/event.

    • Report to your System Administrator for review.
